Rhodium Healthcare Services, Inc is a privately-owned, state-recognized and licensed Home Health Care Agency. We serve a wide variety of community in the Twin Cities Metro Area and beyond. Rhodium Healthcare Services is a premier Home Health Care Agency was founded in 2018 by seasoned healthcare 

professionals with over 50 years of cumulative experience in the business and healthcare industry. Rhodium Healthcare Services is licensed to provide a variety of homecare and nursing services, and is also a licensed assisted living provider. Each of our unlicensed staff is well trained in CPR, first aid, medication administration and areas specific to a client’s medical needs. We are proficient in setting up medical appointments as well as monitoring, tracking and ensuring appropriate follow-ups on medical concerns. Our licensed professionals include Registered Nurse (RN) and Licensed Practical Nurse (LPN) with many years of experience in the home healthcare industry. You are assured of professional and reliable services with our nurses.

AREAS WE SERVE

Our corporate headquarters is located in Brooklyn Center; however, we serve all the 7 counties around the Twin Cities, which comprises Anoka, Hennepin, Ramsey, Dakota, Scott, Carver, and Washington, and beyond. Our assisted living is located in Wright County, hence, we serve clients outside the Twin Cities metro area. We continue to grow our service areas outside the Twin Cities. Through quality and tailored care, education, and support services, Rhodium Healthcare Services, Inc promotes and protects the dignity of seniors and people with disabilities and their families.

AGENCY PHILOSOPHY

We believe that health care is a basic human right which must be made readily available, coordinated, and provided in a comprehensive and customized way.  We believe home health care is an important part of the continuous health care system, and it shall be provided in the most cost-effective way possible.  We believe the services should be tailored to meet the needs and desire of our clients. Rhodium Healthcare Services, Inc shall operate and provide services in compliance with all applicable federal, state, and local laws and regulations and disclosure and ownership information.  To the best of our knowledge, based on our Quality Management Program and professional personnel practices, our services comply with professional standards and principles.

We believe that home health services are an essential part of the health care delivery system.  By providing continuing care to clients in their own homes, home health services minimize the effects of illness, accident, or disability.

The Community Access for Disability Inclusion (CADI) Waiver provides funding for home and community-based services for children and adults, who would otherwise require the level of care provided in a nursing facility.
CADI Waiver services may be provided in a person’s own home, in his/her biological or adoptive family’s home, in a relative’s home (e.g. sibling, aunt, grandparent etc.), a family foster care home or corporate foster care home, a board and lodging facility or in an assisted living facility. If married, a person may receive CADI Waiver services while living at home with his or her spouse.

Who is eligible for CADI Waiver services?

Eligibility for CADI Waiver services is determined through a screening process. To be eligible for CADI Waiver services, a person must meet all these criteria:
• Be eligible for Medical Assistance
• Be certified disabled by the State Medical Review Team or by the Social Security Administration
• Be under the age of 65 years when the waiver is opened
• Be determined to require the level of care provided to individuals in a nursing facility
• Have an assessed need for supports and services over and above those available under other funding sources
• Choose care and services in the community instead of a nursing facility

What else is important for participation in the CADI Waiver?

Once eligibility is determined for participation in the CADI Waiver, certain questions must be asked about services including:
• Are the services necessary to ensure the recipient’s health, welfare and safety?
• Is the service covered by any other funding source, for example, Medical Assistance state plan services, private health care coverage, Medicare, education or Vocational Rehabilitation service funding?
• Have all options been assessed, and does this option meet the individual desires, needs and preferences of the person?
• Is the cost of the service considered reasonable and customary?

These are Home and community-based waiver programs we are licensed to work with:
• Alternative Care, a program that supports certain home and community-based services for older Minnesotans, age 65 years and over, who are at risk of nursing home placement and have low levels of income and assets.
• Brain Injury (BI) Waiver for people with acquired or traumatic brain injuries who need the level of care provided in a nursing facility that provides specialized (cognitive and behavioral supports) services for people with brain injury or neurobehavioral hospital level of care.
• Community Alternative Care (CAC) Waiver for chronically ill and medically fragile people who need the level of care provided in a hospital.
• Community Access for Disability Inclusion (CADI) Waiver for people with disabilities who require the level of care provided in a nursing facility.
• Developmental Disabilities (DD) Waiver for people with developmental disability or related condition who need the level of care provided in an Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD).
